Please note that information submitted through the form below is submitted only to the practitioner with whom you book, and is sent securely through Acuity Scheduling.

Please note that at this time Tanya Kahanoff is NOT accepting new clients. Please check back in January for updates on her schedule.

Our office is participating in the Restrictions Exemption Program. This means that clients who meet the criteria below will able to attend face-to-face sessions in the office (Virtual sessions are always an option).

1. People who have proof of being fully vaccinated (2 or more doses). There is a transitional period between September 20 – October 25 where proof of a single does of vaccine will be accepted provided the dose was administered a minimum of 2 weeks before your session.
2. People who can provide evidence of a recent negative Covid test (within 72 hours of service).
3. Children under the age of 12.
4. People that can provide a medical exemption letter from taking the vaccine

Proof of vaccination includes:
· A picture or paper record of a valid Alberta Health Services
· MyHealth Records, pharmacy, First Nations, or physician immunization record prominently displaying the name, type of vaccine and date of administration
· Canadian armed forces immunization record, displaying the name, type of vaccine and date of administration
· An immunization record from another Canadian Province or Territory, displaying the name, type of vaccine and date of administration
· Valid Government of Alberta Vaccination QR code (when available)

Certain forms are required for certain appointment types. When booking online, forms are required to be filled out to the best of your ability upon booking.

Adult ADHD Assessment
$1200.00 - includes initial consultation, interpretation of results, screening tools, report writing, and feedback meeting.

Adolescent ADHD Assessment
$1200.00 - Includes initial consultation with parents/guardians, initial consultation with adolescent, interpretation of results and screening tools, naturalistic observation (if possible), report writing, feedback meeting.

Booking Policies

Payment is accepted by debit/credit, cash or cheque and is due in full at the time of your booked session.

Cancellation Policy
You may be charged a fee for missed appointments or cancellations without 24 hours notice.

Fees may sometimes be covered by private extended health care insurance available through your employer (e.g., Alberta School Employees’ Benefit Plan, Blue Cross, Great West, Sun Life, etc.)  Each plan is different, so please have the details of your plan available when you call, or at your first appointment. It is your responsibility to seek reimbursement directly from your insurer; we will provide your insurance provider any necessary information. Fees not reimbursed by insurance providers are a tax-deductible health expense.
