Forms must be included with your appointment booking information for the following appointment types:

Certain forms are required for certain appointment types. When booking online, forms are required to be filled out to the best of your ability upon booking.

Professional Fees

Individual Therapy
$180.00 per session

Adult ADHD Assessment
$900.00 - includes initial consultation, interpretation of results, screening tools, report writing, and feedback meeting.

Adolescent ADHD Assessment
$900.00 - Includes initial consultation with parents/guardians, initial consultation with adolescent, interpretation of results and screening tools, naturalistic observation (if possible), report writing, feedback meeting.

Booking Policies

Payment
Payment is accepted by debit/credit, cash or cheque and is due in full at the time of your booked session.

Cancellation Policy
You may be charged a fee for missed appointments or cancellations without 24 hours notice.

Insurance/Coverage
Fees may sometimes be covered by private extended health care insurance available through your employer (e.g., Alberta School Employees’ Benefit Plan, Blue Cross, Great West, Sun Life, etc.)  Each plan is different, so please have the details of your plan available when you call, or at your first appointment. It is your responsibility to seek reimbursement directly from your insurer; we will provide your insurance provider any necessary information. Fees not reimbursed by insurance providers are a tax-deductible health expense.
