Forms must be included with your appointment booking information for the following appointment types:

Certain forms are required for certain appointment types. When booking online, forms are required to be filled out to the best of your ability upon booking.

Adult ADHD Assessment
$1200.00 - the assessment consists of 3 appointments which includes a psychiatric assessment, an ADHD assessment, and the feedback meeting addressing the diagnosis and treatment recommendations.

Adolescent ADHD Assessment
$1200.00 - the assessment consists of 3 appointments, one with parents/guardians, one with the adolescent, and a feedback meeting addressing the diagnosis, treatment recommendations with parents/guardians and adolescent.

Booking Policies

Payment
Payment is accepted by debit/credit, cash etransfer, or cheque and is due in full at the time of your booked session.

Cancellation Policy
Failed appointments and cancellations less than 24 hours will be charged to you at the rate of $200.00. This charge must be paid at the following session.

Insurance/Coverage
Fees may sometimes be covered by private extended health care insurance available through your employer (e.g., Alberta School Employees’ Benefit Plan, Blue Cross, Great West, Sun Life, etc.)  Each plan is different, so please have the details of your plan available when you call, or at your first appointment. It is your responsibility to seek reimbursement directly from your insurer; we will provide your insurance provider any necessary information. Fees not reimbursed by insurance providers are a tax-deductible health expense.
